Extracorporeal cardiac shock wave therapy: first experience in the everyday practice for treatment of chronic refractory angina pectoris.

چکیده
Several alternative therapies have emerged for treating patients with chronic refractory angina pectoris, yet only a few have given rise to sufficiently published data regarding safety and effectiveness. It is imperative to establish an effective and preferably non-invasive therapy for this expanding patient cohort. We report initial experience with cardiac shock wave therapy (CSWT) in the everyday practice for refractory angina. Ten patients with chronic refractory angina pectoris were enrolled for CSWT at our institution. All were symptomatic on minimal activity (CCS class III or IV) inspite of receiving maximally tolerated medical therapy. They showed evidence of myocardial ischemia on exercise Tc SPECT perfusion scans with no possibility for coronary percutaneous intervention or bypass grafting on a recent angiogram. The patients were subjected to 9 CSWT sessions (3 cycles) over 3 months. All patients gave written informed consents. The MODULITH SLC (Storz Medical AG, Kreuzlingen – Switzerland) was used throughout this treatment. The physical principles, mechanism of action and technical details of shock waves and the above-mentioned generator have been described elsewhere [1]. Briefly, this device is able to precisely focus shock waves transthoracically on the desired segment of the heart under the guidance of an in-line 2-D echocardiography. Shock waves are applied in an Rwave triggered manner to avoid ventricular arrhythmias. The shock wave is a mechanical force that induces local shear

Extracorporeal Cardiac Shock Wave Therapy Ameliorates Clinical Symptoms and Improves Regional Myocardial Blood Flow in a Patient with Severe Coronary Artery Disease and Refractory Angina
Different therapeutic options are being used for chronic coronary artery disease (CAD). We report about a 51-year-old female with CAD and refractory angina pectoris despite maximally tolerated medical therapy and after both percutaneous coronary intervention (PCI) and coronary artery bypass grafting (CABG). The patient received cardiac shock wave therapy (CSWT) over a period of 6 month. There w...

Objective(s): The current management of coronary artery disease (CAD) relies on three major therapeutic options, namely medication, percutaneous coronary intervention (PCI), and coronary artery bypass grafting (CABG). However, severe CAD that is not indicated for PCI or CABG still bears a poor prognosis due to the lack of effective treatments. In 2006, extracorporeal cardiac shock wave (SW) the...

AIMS There is a continuing search for new treatment options in patients who suffer from refractory angina pectoris to improve quality of life. Several studies have recently demonstrated promising results by stimulating angiogenesis using extracorporeal shockwave therapy in these patients. The purpose of this study is to quantitatively analyse the effect of extracorporeal shockwave therapy on my...
متن کاملDouble-blind and placebo-controlled study of the effectiveness and safety of extracorporeal cardiac shock wave therapy for severe angina pectoris.
BACKGROUND Low-energy shock wave (SW) therapy has improved myocardial ischemia in both a porcine model and in patients with severe angina pectoris. METHODS AND RESULTS To further confirm the effectiveness and safety of SW therapy, 8 patients with severe angina pectoris were treated with SW therapy in a double-blind, placebo-controlled and cross-over manner. SW therapy, but not placebo, signif...
متن کاملNonconventional therapeutic modalities in refractory angina pectoris
Abstract Despite sophisticated conventional medical treatment and the continued development and improvement of coronary revascularization modalities in recent years, a significant number of patients still suffer from refractory angina pectoris and cannot be successfully managed. Advances in therapeutic options have led to an increase in the average of life expectancy even in such ...
